The hero's cause of death was falling into a fire pit and roasted. This was caused by him being an idiot and practicing with a sword in front of an open fire pit with a fire going and he was also incompetent enough to trip and fall into the fire. Then, he screamed in pain from it, causing rocks to fall on him, knocking him unconscious and trapping him in the fire pit.
As for what could have been done to prevent it, all sorts of things could've been done. Putting out the fire first, since it was strong enough to leave the entire body charred. Not practicing with a sword in front of a fire pit would also be a big help. And maybe waiting until you're vaguely competent to practice with swords in the middle of the night, too. He also could've avoided dying if he didn't make a fire pit in a place where rocks might fall on him if he shouts loud enough. That's at least four different ways he could avoided dying, but this entire situation could have been avoided if the hero wasn't a complete idiot.
